Please make checks for full amount payable to “Living Arts of Tulsa” and mail with completed application to 308 S. Kenosha Ave, Tulsa, Ok 74120.

Enrollment is on a first come basis, so don’t delay. Applications will be processed and all parties notified as they are accepted. We will make our final roster one week in advance of the starting date. However, if the student’s enrollment is withdrawn after the roster point, a $35. matriculation fee will be charged. Upon acceptance, the parent must fill out a medical and trip release form, available at Living ArtSpace.
